As nurse practitioners, you will need to analyze a patient’s risk factors, discover any manifesting complications, diagnose the patient, and manage the patient’s care appropriately, knowing when to refer them to other providers.

Directions

Three patients visit you today for Screening and Immunization (Links to an external site.). Choose one your patients and answer the following questions using the Screening and Immunization Template (Word) (Links to an external site.).

Determine the risk factors that might be placing the patient at risk for illness or exacerbation of a chronic illness.

List three areas of concern in ranked order.

Determine at least three appropriate anticipatory guidance/health promotion counseling needs.

Develop a health promotion plan by identifying at least three primary, three secondary, and three tertiary recommendations (nine total).
