Combine this information with your assessment data to identify the needs of your aggregate. Use one of the following Epidemiology models: Epidemiological Triangle or Theory of Multiple Causation (Web of Causation) and apply them to your population and their health risk. This will include the distribution (Who, When and Where) and determinants (WHY – what are some factors that may have impacted or caused the problem?)
