CASE STUDY
Josie is an adorable ten year old with bright eyes and black hair that falls in ringlets. Her mother and father see her regularly in the evenings but both work day shift jobs. Josie suffers with acute lymphocytic leukemia (ALL). She has been on your assignment each time she has chemotherapy and you have watched her grow over the last couple of years in spite of her grim diagnosis. You have cared for her each time. She is your favorite patient and you are her favorite nurse. Her pediatric oncologist has followed the current guidelines for treatment of ALL including the most recent stem cell transplant. Following the transplant, she developed an infection two months after going home. The doctors conclude that if she does not respond to the antibiotics soon, she will succumb.

Her parents do not want Josie to know the severity of the situation. Her parents are both working today and you are changing her central line dressing. She looks at you and says, “I heard Dr. Mike out in the hallway tell mommy I am going to ‘sah-come’. That means I am going to die, doesn’t it?”
