As a data steward, you have been asked to create a Microsoft Access database to store patient demographics. After completing the database, you will need to create a procedure document or presentation to train other employees on how data should be collected, stored, and shared.

1-Part One – Create Database

Create a Microsoft Access database that includes the following demographic elements:

Patient First Name

Patient Last Name

Patient Middle Initial

Street Address

State

Zip Code

Phone number

Social Security Number

Birthdate

Gender

Emergency Contact Name

Emergency Contact Number

Primary Physician

Once you have created the database with the patient demographic elements, provide fictitious information for three patients for each database element.

2-Part Two – Data Management Procedures

Develop a one page document for data management procedures that:

Explains the importance of data warehouse architecture and how it is used for analysis and reporting.

Illustrates data mining processes to extract meaningful data.

Describes two recommendations with an example for implementing the recommendation for collecting, storing, and/or sharing data.
