Using the performance appraisal parameters and definitions below:

Rate each team member’s performance on a scale of 1-5. A team member rates a 5 only if they completely meet the description provided. Otherwise, the rating for any given parameter should be between 1 and 4.

Provide detailed responses to describe specific contributions and/or challenges for each team member.
